Let's be honest, for a small plastic toothbrush head, these are fairly overpriced.

As for sustainability, you don't need to replace anything but the brush-head itself really, but they force you to buy something much longer, probably just so they can charge more for it. Would people pay $7 each for a tiny brush head?

PB also don't seem to have any in stock, so I'm replacing them now when my last one is running low, but I'll have to wait over six weeks to receive them according to the website - that's pretty disappointing PB, need to manage your stock levels a bit better perhaps?

I've been using these for about 3 years now and very satisfied. I change the brush at the 3 monthly recommended interval, but tbh they are in good shape and would go longer.
